Skilled Nursing Facility (SNF) Ownership Disclosure Due January 1, 2026

For those SNFs that have not yet submitted their Form CMS-855A revalidation application or, as applicable, SNF Attachment, this is an important reminder that you must do so by January 1, 2026. The previous deadline was August 1, 2025.  

Failure to do so could result in the stay or deactivation of your Medicare enrollment.
