AHCA POPULATION HEALTH MANAGEMENT SUMMIT

As you may know, a big area of work for the American Health Care Association (AHCA) is supporting members interested in moving into the population health management (PHM) space.  Specifically, AHCA now has a small staff of in-house experts working with members on building their own Institutional Special Needs plans (I-SNPs), developing integrated provider networks which contract with large insurer Medicare Advantage Plans and Accountable Care Organizations, and AHCA is also working on a PAC/LTC ACO model as well as a new PAC Bundle approach.

With MA penetration rates growing, ACO alignment increasing and a new CJR proposed rule at CMS, now is the time for PAC/LTC providers to move into positions of control in PHM to support its members and advance modernization, AHCA is convening a special meeting.

The AHCA Population Health Management Summit will be held in Washington DC on December 9-10.  With the addition of the new Supplemental Benefits for MA Plans (e.g., they now can delivery adult day, meals on wheels, person care, transportation, etc.) it is believed there is quite a bit of new opportunity to forge innovative partnerships with Aging Network providers (i.e., meals on wheels, adult day, etc.), senior housing providers and others.  The Summit is intended to create a forum for members experienced in PHM, as well as those who are interested, to meet partners who can help them move into PHM, connect with health plans, investors and others!
