FlourishConnect is a collaboration between the University of Louisville Trager Institute and the University of North Carolina Center for Aging and Health.

24 Week Program/1 TeleECHO Session per Week:

  • 60 Minute Case Presentation and Discussion
  • Everyone attends initial 24 weeks
  • Develop 6 Competencies:
    1. Age-friendly and trauma informed care strategies
    2. Special needs of dementia patients
    3. Infectious disease management
    4. Improve vaccine access and decrease hesitancy
    5. Behavioral Health needs
    6. Reduce health disparities & enhance social determinants of health for residents and staff
