Liberty Home Care – ordered by a physician to help patients safely transition from a hospital or rehabilitation facility back to their home. It may also be ordered for patients who are currently at home but have experienced a change in their condition. Care is provided by a skilled nurse, physical therapist, occupational therapist, speech therapist, medical social worker and certified nursing assistant.

Liberty Palliative – provides comfort-consulting services to individuals with chronic or life limiting illness while supporting their families. Care is provided by a nurse practitioner with a focus on relieving pain and symptoms related to the disease process. This service can be provided in conjunction with home health.

Liberty Hospice – provides end of life care to individuals with life-limiting illnesses and focuses on quality of life. Care is provided by an interdisciplinary team and support is given to the patient as well as their family/caregiver.
