REQUESTING A HARDSHIP PAYMENT FROM NC MEDICAID

There is a process through which providers can request a hardship advance from the state. Below is the current policy. If there are any changes in this policy, due to the current crisis, we will communicate them to our members as soon as we have the information.

Individual providers and provider organizations having cash flow issues caused by NCTracks, NCFAST, or other State-controlled matters may request a Hardship Payment Advance (HPA). HPAs are not payments for services rendered but are loans that must be repaid to the State once the State-controlled issues are resolved. Repayment is due whether you remain active or cease operating as a provider.

An applicant for a HPA shall provide the following items to NC Medicaid Finance:

  1. An official request on provider letterhead, signed by an authorized individual, providing:
    1. A specific dollar value request for a Hardship Payment Advance:
    2. A description of the issue causing the cash flow issue
    3. The source of the funds that will be used to make the required repayment
  2. A statement attesting that there are no outstanding liens or levies with any Federal or State agency (i.e., tax authorities or Department of Commerce) Contact Info: person’ name, title, and telephone number
  3. Billing NPI number(s) and provider name
  4. Service Address:
  5. Estimate of the percentage Medicaid reimbursement represents of your total cash flow:
  6. A list by recipient of unbilled services dollars, and your estimation of what Medicaid would have paid you for these services
  7. Desired method to receive the advance: a. If by FedEx, provide delivery information including telephone number for driver. b. If by pickup from the NC DHHS Office of the Controller on Dix Campus in Raleigh, provide the name of the person who will come for the check. The Controller will release the check only to this person.

Hardship Payment Advances are calculated at 70 percent of the average provider payments.

Once the HPA is approved, the authorized individual must sign and return a repayment agreement for the advance.
