Participation Needed for the IHI’s Action Community

The Institute for Healthcare Improvement’s (IHI) Action Community is seeking nursing home participation in the program that begins in March.    

The Action Community is a free virtual learning opportunity for nursing homes join other healthcare systems over seven months to accelerate the adoption of the 4Ms Framework. The 4Ms (What Matters, Medication, Mentation, and Mobility) are a set of evidence-based practices intended to make the complex care of older adults manageable, consistent, and reliable. Nearly 700 nursing homes are already recognized as Age-Friendly and have reported positive results for residents and staff.
