Earn a Certificate of Completion for Alliant’s Readmission Improvement Initiative

Long term care facilities can earn a Certificate of Completion for Alliant Health Solutions’ Readmission Improvement Initiative. Alliant will begin awarding certificates in August. Here is how to earn a certificate in five easy steps:   

Email your complete package along with a summary (short paragraph) of what you learned from your readmission work and share any best practices to Marilee Johnson, State Quality Manager, at marilee.johnson@allianthealth.org.  

An 8-Weeks to a Better Readmission Rate initiative began in May. The initiative offers best practice tools and resources to help you reduce readmissions and avoid emergency transfers at your facility. Fill out the interest form to learn more.
