CMS UPDATES 672 AND 802 SURVEY FORMS

As of December 1, 2012 there are updated CMS-672 and CMS-802 forms that will be utilized. The CMS-672 form is given to skilled nursing facilities on each Certification or Recertification survey and contains updated language regarding F tags F133 to F139 which concern residents that are receiving psychoactive medications.

The CMS- 802 is an entirely different form. Because North Carolina is a QIS state, facilities will no longer be asked to provide a CMS-802 during QIS Certification/Recertification Surveys or QIS Follow-Up Surveys. Please note, the CMS-802 could be used during a Complaint Investigation Survey and a survey team might ask a facility to complete one. If a facility does not have a CMS-802 the survey team will provide a copy. Most MDS programs will generate this document. The changes on the CMS-802 also update information under Psychoactive Medications and Behavior Symptoms.

Palmetto GBA is now offering the next stage of Online Provider Services (OPS) that includes secure messaging and secure forms. This new feature was implemented on October 25, 2012 for J11 A/B MAC providers. If you are a J11 A/B MAC Provider Administrator, you will notice a Messaging/Forms tab as well as a message box on your Palmetto home page.

Secure forms are the first phase of this enhancement. You now have the ability to submit a first-level appeals form, along with attachments, through OPS. Once the form is submitted, you will receive a message in your inbox. You will get a second message with the Document Control Number (DCN) when the form has begun to be processed. You can use the DCN to look up form processing status and view your submitted forms.

NEED TO GO

Record flooding and power outages across the northeastern United States made for a long night caring for the most critically ill, as several hundred patients were evacuated in New York City, day-time hospital staff slept overnight on vacant beds and less urgent procedures were postponed.
